Dogs and especially puppies like to explore their surroundings by tasting/eating inappropriate things.
Symptoms of chocolate poisoning depend on the theobromine concentration in the specific type of chocolate, the amount consumed by the animal and the weight of the animal - and base on the info you gave me - I suspect that this is a case of an intake above the toxic level.

Untreated poisoning can lead to serious, even fatal consequences. The symptoms often appear between 2-24 hours after intake, and can be:
- Decreased general condition
- Trembling, weakness and loss of balance
- Vomiting and diarrhoea
- arrhythmic increased heart rate
- Increased body temperature
- Salivation and nausea
- Cramps
Symptomatic treatment is often given at the vet, by forex: induce vomiting if necessary, rinse the dog's stomach, coal and IV fluids and other medical treatments.
